****SERVICES AND BRIEF DESCRIPTIONS****

Intensive In Home, Community Support Team, Clinicial Assessments, Medication Management, Outpatient Therapy, Group Therapy, Psychosocial Rehabilitation, Substance Abuse Intensive Outpatient, Target Case Management, Home Care Services

PSYCHOSOCIAL REHABILITATION (PSR)(ADULTS)

These services are for adults 18 and older. This is a program that focuses on skill and resource development related to life in the community and to increasing the member’s ability to live as independently as possible.  This program is available for a minimum of 5 hours or more a day at least 5 days a week and is provided on weekends or in the evening.

This program teaches:

A. community living- such as housekeeping, shopping, cooking, use of transportation facilities, money management;

B. personal care such as health care, medication self-management, grooming;

C. social relationships;

D. use of leisure time

E. educational activities which include assisting in securing needed education services such as adult basic education and special interest courses; and

F. prevocational activities which focus on the development of positive work habits and participation in activities that would increase the participant’s self worth, purpose and confidence; these activities are not to be job specific training.

Substance Abuse Intensive Outpatient Program (SAIOP) (Children and Adults) 

This is a structured individual and group addiction service designed to assist adults and adolescents members to begin recovery and learn skills for recovery maintenance.  This program is offered at least 3 hours a day at least 7 days a week. 

SAIOP shall include a structured program consisting of, but not limited to the following services:

1. Individual counseling and support;

2. Group counseling and support;

3. Family counseling, training or support;

4. Biochemical assays to identify recent drug use (e.g. urine drug screens);

5. Strategies for relapse prevention to include community and social support systems in treatment;

6. Life skills;

7. Crisis contingency planning;

8. Disease Management; and

9. Treatment support activities that have been adapted or specifically designed for persons with physical disabilities, or persons with co-occurring disorders of mental illness and substance abuse/dependence or mental retardation/developmental disability and substance abuse/dependence.

Medication Management (Children and Adults)

Through this service the members are educated, questions answered, concerns, preferences, wants and needs as they relate to medications are answered by the physician providing medicaiton management service.  The members gain confidence in his/her medications with the physician thereby leads to improved adherence and persistence.  Ultimately, the members benefit from the medication management services through improved overall health outcomes. 

Clinical/Behavioral Assessments (Children and Adults)

A Clinical/Behavioral Assessment is an intensive clinical and functional face to face evaluation of a recipient’s mental health, developmental disability, or substance abuse condition that results in the issuance of a Diagnostic/Assessment report with a recommendation regarding whether the recipient meets target population criteria, and includes an order for Enhanced Benefit services that provides the basis for the development of an initial Person Centered Plan.

Outpatient Therapy (Children and Adults)

Assessment and ongoing outpatient treatment for individuals is provided by licensed mental health professionals to assist the individual in developing strategies to plan for wellness and to recognize triggers and early warning signs.  Therapy services can be provided in the office, school, home or community. Prior to services a thorough assessment of the individual’s strengths and needs is completed.

Therapists provide clinical consultation on how to best build a treatment plan that will address the functional deficits that are directly related to the individual’s mental health needs.

Home Care Services (Children and Adults)

Our Home Care Services provides the following services:

1.  Provide or assist with personal care (assisting with bed mobility, bath and shower, toileting, dressing, eating and with continence needs)

2.  Assist with ambulation: help with prescribed exercises which the client and in-home aides have been taught by a licensed heath care practitioner.

3.  Assist client with self-administration of medications which are ordered by the physcician or other person authorized by state law.

4.  Record and report changes in the client’s condition, family situation, or needs to an appropriate health care practitioner.
